CHECKLIST <br /> Before returning your signed grant agreement, piease double-check the following items <br /> to assure completion: <br /> Verify the Grantee's correct federal taxpayer identification number (FEIN) on page <br /> 1 of the Notice of Grant Award. <br /> Verify the Grantee's correct legal/business status in the appropriate blank on <br /> page 2 of the Notice of Grant Award. <br /> The authorized official namecl on page 2 of the Notice of Grant Award must sign <br /> the grant agreement. (If you have any questions about who is authorized to sign, <br /> please call DCCA before proceeding.) The same authorized official must sign the <br /> acknowledgment on the last page of Part II. . <br /> Make any necessary corrections to the Grantee's address on page 2 of the Notice <br /> f r <br /> o G ant Award. <br /> Fill in the name, phone number, and address if different from the Grantee's <br /> ( <br /> address) for the administrator/project manager, on page 2 of the Notice of Grant <br /> Award. The authorized officia/must a/so sign in this area if an administrator or <br /> project manager is being designated. By doing so, the designated person will be <br /> allowed to sign future grant correspondence and quarterly and final reports. ' <br /> If the Grantee is not a government entity, the Reporting Requirements in Part II, <br /> Section 2.5 (a) require that you provide "authenticating documentation" at the <br /> time you return the grant agreement to DCCA. (If you have any questions about <br /> what constitutes "authenticating documentation," please call DCCA before <br /> proceeding.) <br /> To reiterate, the same authorized official must sign the acknowledgment on the <br /> last page of Part II. <br /> Review Part VI, Section 6.6 (Drugfree Workplace Act) carefully and mark the <br /> statement with an "X" as it relates to the Grantee. <br /> Remove the reporting forms and retain th.em for your future use. <br /> Return the signed Notice of Grant Award and Parts I through VI and Exhibit I to <br /> your DCCA Contact: <br /> Kathy Rudolph � <br /> Illinois Department of Commerce and Community Affairs <br /> 620 E.
