Anderson Plaza <br /> Decatur, Illinois 62523-1196 <br /> Attention: Kris Quandt <br /> Gentlemen: <br /> This Letter Permit is your authorization for the City of Decatur to close <br /> lanes on Business Route 51 (Main Street and Water Street), Illinois <br /> Route 48 (Fairview Avenue and Oakland Avenue), Illinois Route 121 <br /> (Pershing Road), Illinois Route 105 (Lake Shore Drive), and U.S. <br /> Route 36 (Eldorado Street) for a manhole inspection project. <br /> This Letter Permit is being granted to the following provisions: <br /> 1. Traffic Control for the proposed lane closures shall be in <br /> accordance with the applicable portions of Highway Standard <br /> 701426-01 except (1) sign spacing shall be 200 feet, (2) a flagger, <br /> two road construction ahead signs, and finro flagger signs will be <br /> required when working in the center lane of a multi-lane route, and <br /> (3) a flagger, a road construction ahead sign, and a flagger sign for <br /> each direction will be required when working in a bi-directional or <br /> dedicated turn lane. <br /> 2. There shall be no additions or changes of location in the work <br /> authorized by this permit without prior approval of the District <br /> Engineer or his duly authorized representative. <br /> 3. The Applicant shall locate, furnish, construct, and maintain the <br /> entire improvement at their expense and shall assume all risk or <br /> liability of every nature accruing during construction or thereafter as <br /> a result of the improvement. <br />
