Anderson Plaza <br /> Decatur, IL 62523 <br /> Re: Supplemental Agreement No. 1 <br /> Sediment Basin Project (HLC Project No. 4572.02) <br /> Dear Keith: <br /> Per our recent conversations,we herein request Supplemental Agreement No. 1 for the Lake <br /> Decatur Basin 6 Sediment Storage Site Engineering Services project due to additional work to be <br /> performed beyond the scope of work agreed to in our original contract. Our proposal is attached <br /> for your review and approval. <br /> The additional work is for the development of a Mobilization Site for the Basin 6 Dredging <br /> Project, including roadway, supply storage area,workshop building,parking, and a docking <br /> facility to support the activities necessary to the dredging of Basin 6. Deliverables will include <br /> concept plans and cost estimates, preliminary plans, final plans and specifications, assistance with <br /> the bidding process, construction observation and layout services, and project administration. <br /> Please see Attachment A for a breakdown of our staffing and cost estimate. <br /> We propose that payment for our services be on a schedule of rates basis, the same basis being <br /> used for the existing Engineering Services Agreement. We estimate the fee for this additional work <br /> will not exceed $ 78,352, which will include all labor, direct costs,profit and work for our sub- <br /> consultants consisting of lighting design and soils analysis. This supplement will increase our <br /> original upper limit of compensation from $ 769,954 to $ 848,306. <br /> We would appreciate your attention to this matter at your earliest convenience.
