t r <br /> t <br /> AGREEMENT FOR ENGINEERING SERVICES <br /> FOR THE <br /> CTTY OF DECATUR <br /> REHABILITATION OF THE 25`� STREET SEWER <br /> TffiS AGREEMENT, made and entered into this �� day of September, 2001, by and <br /> between the CITY OF DECATUR, ILLINOIS, hereinafter referred to as the "CIT'Y" and <br /> BAINBRIDGE, GEE, MII,ANSKI & ASSOCIATES, INC., 1670 South Taylorville Road, <br /> Decatur, Illinois 62521, hereinafter referred to as the"ENGINEER", covers professional engineering <br /> services in connection with the preparation of a rehabilitation report for the 36-inch 25�' Street Sewer, <br /> hereina.fter referred to as the "PROJECT". <br /> Said PROJECT, commencing at the intersection of Lake Shore Drive and 16�` Street <br /> (Manhole No. 59-17), extends northeasterly approximately 11300 feet to the intersection of Eldorado <br /> Street and 25"` Street (Manhole No. 34-23). <br /> THEREFORE, in consideration of these premises and of the mutual covenants herein set <br /> forth, <br /> THE ENGINEER AGREES: <br /> 1. Desi� Concept Report - To perform the following engineering services in the preparation <br /> of a report recommending a rehabilitation strategy. <br /> a. Review videotapes of an internal survey of the sewer as prepared by Telescan, Inc. <br /> and as provided by the CIT'Y and make a detailed log of all defects noted. <br /> b. Perform an internal survey of a11 manholes contained within the limits of the <br /> PROJECT. Access to any buried manhole to be provided by the CTTY. <br /> c. Perform an internal survey of the sewer in the vicinity of select manholes to verify <br /> information from the tapes. <br /> d. Research laboratory records of the Sanitary District of Decatur to ascertain pH, <br /> temperature, hydrogen sulfide content of the ef�luent with the PROJECT. <br /> e. Review available information about the sewer including condition surveys, reports, <br /> original construction drawings, etc. i <br /> f.
