t r t AGREEMENT FOR ENGINEERING SERVICES FOR THE CTTY OF DECATUR REHABILITATION OF THE 25`� STREET SEWER TffiS AGREEMENT, made and entered into this �� day of September, 2001, by and between the CITY OF DECATUR, ILLINOIS, hereinafter referred to as the "CIT'Y" and BAINBRIDGE, GEE, MII,ANSKI & ASSOCIATES, INC., 1670 South Taylorville Road, Decatur, Illinois 62521, hereinafter referred to as the"ENGINEER", covers professional engineering services in connection with the preparation of a rehabilitation report for the 36-inch 25�' Street Sewer, hereina.fter referred to as the "PROJECT". Said PROJECT, commencing at the intersection of Lake Shore Drive and 16�` Street (Manhole No. 59-17), extends northeasterly approximately 11300 feet to the intersection of Eldorado Street and 25"` Street (Manhole No. 34-23). THEREFORE, in consideration of these premises and of the mutual covenants herein set forth, THE ENGINEER AGREES: 1. Desi� Concept Report - To perform the following engineering services in the preparation of a report recommending a rehabilitation strategy. a. Review videotapes of an internal survey of the sewer as prepared by Telescan, Inc. and as provided by the CIT'Y and make a detailed log of all defects noted. b. Perform an internal survey of a11 manholes contained within the limits of the PROJECT. Access to any buried manhole to be provided by the CTTY. c. Perform an internal survey of the sewer in the vicinity of select manholes to verify information from the tapes. d. Research laboratory records of the Sanitary District of Decatur to ascertain pH, temperature, hydrogen sulfide content of the ef�luent with the PROJECT. e. Review available information about the sewer including condition surveys, reports, original construction drawings, etc. i f.
