' . �' <br /> � , � <br /> � � <br /> HOME PROGRAM FY2001-02 <br /> SUPPORTIVE HOUSING AGREEMENT <br /> THIS AGREEMENT, entered into on the Z� day of �,c- ✓J� , 2001 by and between <br /> the City of Decatur, Illinois,hereinafter called the "City", and e��Behavioral Health Center, <br /> Inc., hereinafter called"Heritage, Inc.", organized under the laws of the State of Illinois. <br /> WHEREAS, the City has entered into an agreement with the U.S. Department of Housing and <br /> Urban Development, hereinafter called "HUD", for the purpose of conducting a HOME Program, <br /> and; whereas the City may commit funds for HOME funds for supportive housing projects; and <br /> whereas Heritage Inc. submitted a proposal dated December 18, 2000 to conduct activities <br /> eligible under the HOME program guidelines and requested a grant of HOME funds not to <br /> exceed$50,000 to fund the activities under the stated proposal. <br /> NOW, THEREFORE, the parties in consideration thereto mutually agree and convenant as <br /> follows: <br /> I. SCOPE OF SERVICE <br /> A. Heritage Inc. may receive up to $50.000 from the City under the HOME Program to <br /> implement the proposed activities. Funding will be dispersed based upon actual project <br /> expenditures, for a total not to exceed the grant amount of$50,000. <br /> B. Heritage Inc. shall use the funds as outlined in the proposal dated December 18, 2000 <br /> for eligible costs of predevelopment, acquisition, rehabilitation, construction, soft costs <br /> and other eligible items associated with the proj ects. All additional sources of funds for <br /> the project must be confirmed prior to the start of the project. <br /> C. Heritage Inc. shall not obligate or expend any funds under the terms of this agreement <br /> until a written Notice to Proceed is issued by the Neighborhood Services Department- <br /> Neighborhood Renewal Division of the City. <br /> II. REPORTING AND REIMBURSEMENT PROCEDURE <br /> A. Progress Reports <br /> Heritage Inc. shall provide the City with quarterly progress reports and financial reports <br /> until the HOME program funds are expended. Quarterly reports shall be submitted to the <br /> City by the tenth(lOth)working day of the succeeding calendar month. Quarterly reports <br /> shall include the status of the project, financial expenditures to date, any anticipated <br /> revisions to the project budget, and any other pertinent information. The City reserves the <br /> right to request additional information as necessary before processing reimbursement <br /> requests. Reimbursements will only be processed if Heritage Inc. is current with '� <br /> submissions of quarterly reports. I� <br /> 1 <br />
