Section 3. That conditional use permit be, and the same is hereby, expressly conditioned <br /> as follows: <br /> (a) That the proposed operation be limited to a maximum of twelve (12) <br /> children; <br /> (b) That a drop-off/pick-up area shall be required along North Gulick <br /> Avenue; <br /> (c) That the proposed operation be in compliance with applicable <br /> provisions of the City's adopted Building Codes, including obtaining a building <br /> permit to perform any proposed or required improvements to the existing <br /> dwelling; <br /> (d) That the proposed daycare facility be at all times appropriately <br /> licensed by the State of Illinois, and a copy of that license, upon approval, shall be <br /> filed with the Economic and Urban Development Department; <br /> (e) That the State Fire Marshal shall be contacted regarding a field <br /> inspection and a copy of said fire inspection report, upon completion, shall be <br /> filed with the Decatur Fire Department; <br /> (fl That a vented hood system shall be required for the cooking facilities; <br /> and, <br /> (g) That the fence shall be repaired, and include a secured entrance from <br /> the alleyway. <br /> Section 4. That said conditional use permit granted hereby, be, and it is hereby, made <br /> subject to the applicable provisions and requirements of Ordinance No. 3512, as amended, the <br /> same being the Zoning Ordinance of the City of Decatur, Illinois, and that the Zoning Map of <br /> said City be amended to show the granting and existence of this conditional use permit. <br /> PRESENTED, PASSED, APPROVED AND RECORDED this 21 st day of October, <br /> 2002. <br /> T M.
