R2000-207 ' <br /> RESOLUTION AUTHORIZING APPLICATIONS <br /> FOR GROUP HEALTH INSURANCE COVERAGE <br /> BE IT RESOLVED BY THE COUNCIL OF THE CITY OF DECATUR, ILLINOIS: <br /> Section 1. T'hat the Benefit Plan Application for the City of Decatur Group Health <br /> Self-insurance Program, and the application for Stop Loss coverage from Consociate Group for <br /> the period November 1, 2000, through October 31, 2001, presented to the City Council herewith <br /> be, and the same are hereby, approved, subject to the approval of Decatur PBPA Labor Committee <br /> and IAFF Local 505, as required by the collective bargaining agreements with these unions. In the <br /> event that agreement cannot be reached with these unions, that the Benefit Plan Application for <br /> the City of Decatur Group Health Self-insurance Program, and the application for Stop Loss <br /> coverage from Health Care Services, Inc., for the period November 1, 2000, through October 31, <br /> 2001,presented to the City Council herewith be, and the same are hereby, approved. <br /> Section 2. If pursuant to Section 1 hereof, the Benefit Plan Application and Stop Loss <br /> coverage from Consociate Group are approved by the unions named therein, then the City <br /> Manager and the City Clerk are authorized and directed to sign, seal and attest a Discount <br /> Provision Agreement with Hospital Sisters Health Systems (St. John's Hospital, Springfield, <br /> Illinois). <br /> Section 3. That the City Manager and City Clerk be, and they are hereby, authorized and <br /> directed to sign and forward said applications on behalf of the City as stipulated in Section 1. <br /> Section 4.
