7. The City shall pay the voucher within 35 days after the voucher and the properly <br /> completed Cab Ride Forms and trip sheets are received from the Cab Company. <br /> 4. The City shall have the responsibility for establishing the guidelines for the Program. <br /> 5. The City shall register Program participants, determine eligibility, and furnish proper <br /> identification to eligible participants. <br /> 6. The City shall provide adequate supplies of Cab Ride Forms to individual participants, social <br /> agencies, the Cab Company, and other interested parties. <br /> 7. The City shall promote the Program via radio,newspaper, informational brochures, and <br /> various other media as the City deems necessary. <br /> 8. The City shall determine the cash fare to be paid by Program participants and reserves the <br /> right to change that rate. <br /> 9. 'The Cab Company shall be excused for failure to perform services under this Agreement if <br /> said service is prevented by reason of Acts of God, strikes, labor disputes, or other occurrences <br /> over which the Cab Company has no control. <br /> 10. If during the course of this Agreement,the Cab Company fails or refuses to provide <br /> transportation services for the Program for reasons other than those specified above, the City ; <br /> may at its option assess the Cab Company an amount not to exceed $100.00 per day as damages , <br /> from the date of discontinuance of service to the date that service is reinstituted. <br /> 11. This Agreement may be modified periodically by mutual agreement of both parties in order <br /> to meet the changing transit needs of Decatur or to create greater operating efficiency. <br /> 12. This Agreement shall terminate if the franchise or the license of the Cab Company to operate <br /> taxi-cab service in the City is suspended or revoked. <br /> 13. The City reserves the right to terminate this Agreement prior to the time specified herein if <br /> there is a reduction or loss of federal funding that supports the Program, provided that the Cab <br /> Company is notified sixty(60) days in advance of such action. <br /> 14. For purposes of this Agreement, notes and other communications shall be addressed to the <br /> respective parties as follows: <br /> The Citv The Cab Company <br /> The City of Decatur, Illinois Pride Midwest Services, Inc. <br /> #1 Gary K.
