*, <br /> AGREEMENT <br /> This Agreement is made and entered into by and between the CITY OF DECATUR, ILLINOIS, ��, <br /> a municipal corporation hereinafter referred to as the"City", and DECATUR TAXI, I <br /> INCORPORATED, an Illinois corporation authorized to do and doing business in the City of �, <br /> Decatur,hereinafter referred to as the"Cab Company". ' <br /> WHEREAS the City and the Cab Company entered into an agreement, executed by the City on ' <br /> Apri16, 1999, with an expiration date of Apri130, 2000; and '� <br /> WHEREAS on May 1, 2000, the City offered the Cab Company an agreement to continue to <br /> provide services for an additional six(6)months,but the Cab Company declined the offer; and <br /> WHEREAS the Cab Company is now willing and able to accept the responsibilities of this <br /> Agreement, and has requested that the City award them this Agreement; <br /> NOW THEREFORE, for and in consideration of the promises, covenants, agreements and I <br /> conditions hereinafter set forth to be performed,kept and met by the respective parties hereto, <br /> this Agreement, hereinafter referred to as the"Agreement",is made by and between the CITY <br /> OF DECATUR, ILLINOIS, and DECATUR TAXI, INCORPORATED, as follows: <br /> 1. This Agreement shall become and be effective as of October 1, 2000, and shall continue in <br /> effect through Apri130, 2005, or until one party or the other shall give notice of its intention and <br /> desire to terminate the same, not less than ninety(90) days prior to the desired date of <br /> termination. <br /> 2. Commencing with the first day of the term of this Agreement and continuing through the <br /> duration thereof, the Cab Company agrees to furnish taxi-cab transportation for those persons <br /> who are eligible participants in the City's subsidized taxi-cab program for the disabled, <br /> hereinafter referred to as the"Program". <br /> 3. The Cab Company shall provide transportation services adequate to meet the needs of the <br /> City's Program and shall operate in accordance with the guidelines issued by the City, which <br /> shall include,but not be limited to, the following conditions: <br /> A. Pro�ram Requirements: <br /> 1. The Cab Company shall only transport Program participants who present a Decatur <br /> Public Transit System photo-identification card. <br /> 2. The Cab Company shall only transport Program participants who are residents of the <br /> City of Decatur, Illinois, and whose trip origin and destination are within the service area <br /> of the Decatur Public Transit System. <br /> 3. Program services shall be on a shared ride basis and the Cab Company shall match as <br /> many rides as is practicable. <br /> 4. Rides shall be scheduled a day in advance to facilitate shared riding and to assist in <br /> scheduling and dispatching. <br /> - 1 - <br />
