V SUBURBAN RESPONSIBILITIES <br /> 5.1 SUBURBAN shall maintain an office with regular business hours and with sufficient <br /> staff to receive and respond to telephone calls from the CTTY or the public regarding leaf <br /> vacuuming questions or complaints, and shall designate a contact person for the CITY. <br /> SUBURBAN shall publish the telephone number of said office in telephone directory of general <br /> circulation. <br /> 5.2 SUBURBAN shall promptly investigate and courteously resolve all complaints, <br /> including but not limited to those involving missed leaf vacuuming services. SUBURBAN and <br /> the CITY agree jointly to establish reasonable administrative regulations for the investigation and <br /> resolution of complaints. <br /> 5.3 SUBURBAN shall maintain a daily log of complaints received using a complaint <br /> form provided by the CITY, which form shall include the name, address and telephone number <br /> of the complainant, complaint, date of complaint, SUBURBAN'S response and the date resolved. <br /> A copy of this form shall be given to the City at the end of each month. <br /> 5.4 Each vehicle used by SUBURBAN for leaf vacuuming shall be equipped with <br /> warning flashers, warning alarms to indicate movement in reverse and signs on the rear stating <br /> "This Vehicle Makes Frequent Stops". <br /> 5.5 All vehicles used by SUBURBAN for leaf vacuuming shall be maintained as <br /> required by all applicable laws, ordinances and regulations. All such vehicles shall be <br /> maintained in good working order and appearance, shall be kept free of odor or rust, and shall be <br /> clean at the start of each collection day. All SUBURBAN vehicles shall display the name of <br /> SUBURBAN, a local telephone number and a vehicle identification number on each side. <br /> 5.6 No vehicles used for leaf vacuuming shall be overloaded nor shall such vehicles be <br /> parked in any one place while loaded any longer than is necessary to collect the leaves of the <br /> location being served. Should any significant amount of leaves blow off a vehicle for any reason, <br /> SUBURBAN shall be responsible for collecting them; if SUBURBAN does not do so, the CITY <br /> may collect and dispose of them and bill the cost thereof to SUBURBAN. All vehicles shall be <br /> made available for inspection by the CITY during regular business hours, at the request of the <br /> CITI'. <br /> 5.7 At all times during the term hereof, SUBURBAN shall keep on file with the CITY <br /> the name and address of the disposal facility at which the leaves collected hereunder are <br /> disposed, which facility sha11 operate in accordance with all applicable IEPA rules and <br /> regulations. SUBURBAN shall permit the CITY to inspect all receipts or disposal tickets from <br /> such facility, at the City's request. <br /> 5.8 SUBURBAN shall keep accurate records of the total volume, or weight, of leaves <br /> collected and the level of participation by the public, as measured by the number of premises <br /> 3 <br />
