Anderson Plaza <br /> Decatur, Illinois 62523 <br /> Dear Mr. McNabb: <br /> Attached are approved copies of Highway Permit Number 5-27283 for your files. We again wish to call your <br /> attention to the necessity for using safety precautions and proper traffic control during the course of <br /> construction. <br /> The applicant or their contractor is required to accurately complete the enclosed form BT 725 and return it to <br /> this office at least one week prior to construction as stated in the "Traffic Control Statement" in the permit. <br /> This completed form will be notification to the Department as to when you will be working and that proper <br /> traffic control shall be used as required in the permit. The applicant or their contractor will not be allowed to <br /> work on State right-of-way until an accurately completed BT 725 form is filed in the District office. <br /> Provisions of this permit require inspection and approval of the work by our District Engineer when <br /> completed, and you are held liable in accordance with the terms of the permit until so approved. <br /> Attached is a form letter for your use in notifying our District Engineer that you request his inspection and <br /> approval. Please return the letter when the permit work is completed. <br /> Please note this permit is valid for a period of 90 days. An extension of time will be granted on request to <br /> this office when supported by legitimate reasons causing delay. <br /> Should any questions arise as to details of how the work should be done, we suggest you call Permit <br /> Supervisor, Mr. Robert A.
