This permit is required to be available at the job site at all times for <br /> inspection by a duly authorized representative of the Department of Transportation or <br /> the Illinois State Police. <br /> Upon completion of all construction or maintenance work on State highway rights-of- <br /> way, the Contractor and/or the Applicant shall remove all excess material and restore all <br /> turf and terrain to the satisfaction of the Department. Such cleanup and repair may <br /> consist of backfilling, regrading, reseeding, resodding, or any other requirements to <br /> restore the right-of-way to a condition equivalent to that which existed prior to the <br /> commencement of the project. , <br /> No deviations to the work specified in this permit will be allowed without approval from , <br /> this office. If the Applicant deviates without approval, the facility may be required to be I <br /> relocated off State right-of-way at the Applicant's expense. <br /> The work authorized by this permit shall be coordinated with the State of Illinois' <br /> Resident Engineer for the proposed roadway construction. The work shall also be <br /> coordinated with other utilities making adjustments. <br /> The Applicant or his Contractor (or Contractors) shall control and protect all vehicular <br /> and pedestrian traffic by use of signs, barricades, flagmen, lights and watchmen and by <br /> any other means as required in the "Manual on Uniform Traffic Control Devices for <br /> Streets and Highways" during the progress of the work as described within this permit. <br /> Minimum desirable standards are attached for normal situations. However, additional <br /> protection must be provided when special complexities and hazards arise. <br /> The Applicant, his successors and assigns, agrees to hold harmless the State of Illinois <br /> and its duly appointed agents and employees against any action for personal injury, <br /> property damage, or claims arising out of environmental laws or regulations sustained <br /> by reason of the exercise of this permit. <br /> The Applicant must ascertain the presence of Highway Authority Agreements <br /> established in accordance with 35 III. Admin. Code Section 742.1020 in the path of its <br /> proposed installation and take precautions to protect its workers, human health and the <br /> environment in those areas. (See Section 530.2340). Where contamination is <br /> encountered through excavation in the right-of-way, it should be managed off site and <br /> IDOT's generator number for the appropriate county may be used. <br />
