, . <br /> AGREEMENT <br /> For and in consideration of the promises, covenants, agreements and conditions hereinafter set <br /> forth to be performed, kept and met by the respective parties hereto, this Agreement is made by <br /> and between the CITY OF DECATUR, ILLINOIS, a municipal corporation hereinafter referred <br /> to as "City", and PRIDE MIDWEST SERVICES, INCORPORATED, d/b/a DECATUR PRIDE <br /> CHECKER CAB an Illinois corporation authorized to do and doing business in the City of <br /> Decatur, hereinafter referred to as the"Cab Company", as follows: <br /> '� DIBIA �ECA�UR yt��OW CN�CKf�RCAg <br /> 1. This Agreement shall become and be effective as of November 1, 1999, and shall continue in <br /> effect until Apri130, 2000; thereafter, the Agreement may continue in effect from year to year, or <br /> until one party of the other shall give notice of its intention and desire to terminate the same, not <br /> less than ninety (90) days prior to the first day of May in the year in which it is sought to cause <br /> said termination. <br /> 2. Commencing with the first day of the term of this Agreement and continuing through the <br /> duration thereof, the Cab Company agrees to furnish taxi-cab transportation for those persons <br /> who are eligible participants in the City's Subsidized Taxi-Cab Program for the Disabled, <br /> hereinafter referred to as the "Program". �� <br /> 3. The Cab Company shall provide transportation services adequate to meet the needs of the <br /> Decatur Program and shall operate in accordance with the guidelines issued by the City, which <br /> shall include, but not be limited to, the following conditions: <br /> A. Program Requirements: <br /> 1. The Cab Company shall only transport Program participants who present a Decatur <br /> Public Transit System photo-identification card. <br /> 2. The Cab Company shall only transport Program participants who are residents of the <br /> City of Decatur, Illinois, and whose trip origin and destination are within the City limits. <br /> 3. Program services shall be on a shared ride basis and the Cab Company shall match as <br /> many rides as is practicable. <br /> 4. Rides shall be scheduled a day in advance to facilitate shared riding and to assist in <br /> scheduling and dispatching. <br /> 5. The Cab Company shall collect a cash fare of seventy-five cents ($0.75) from each <br /> eligible rider and seventy-five cents ($0.75) from each escort accompanying an eligible <br /> Program participant. Personal Care Attendants shall ride for free. The fares collected are <br /> Program revenues and shall be remitted to the City at the end of each month. <br /> - 1 - <br />
