R99-108 <br /> RESOLUTION AUTHORIZING APPLICATIONS <br /> FOR GROUP HEALTH INSURANCE COVERAGE <br /> BE IT RESOLVED BY THE COUNCIL OF THE CITY OF DECATUR, ILLINOIS: <br /> Section 1. That the Benefit Plan Application for the City of Decatur Group Health <br /> Self-insurance Program, and the application for Stop Loss coverage from Health Care Services <br /> Corporation for the period November 1, 1998, through October 31, 1999, presented to the City <br /> Council herewith be, and the same are hereby, approved. I <br /> Section 2. That the City Manager be, and he is hereby, authorized and directed to sign , <br /> and forward said applications on behalf of the City. �'�, <br /> Section 3. That the City Manager be, and he is hereby, authorized and directed to sign the <br /> Administrative Services Agreement with Health Care Services Corporation for the period <br /> November 1, 1998, through October 31, 1999, presented to the Council herewith, on behalf of <br /> the City. <br /> PRESENTED AND ADOPTED this 12th day of July, 1999. <br /> ,�;-- <br /> T .
