� <br /> i <br /> ' • <br /> � <br /> Illinois Department of Transportation <br /> Division of Public Transportation <br /> 310 South Michigan Avenue/ 16th Floor/ Chicago, Illinois/60604-4205 <br /> TO: All Participants in the <br /> Downstate Operating Assistance Program <br /> FROM: Dale Fitschen <br /> SUBJECT: Fiscal Year 2000 (FY00) Operating Assistance Application Forms <br /> CATE: March 1, 1999 � <br /> Enclosed please find the application forms for Fiscal Year 2000 which I� <br /> must be filed in connection with the Operating Assistance Program. <br /> The FY00 forms reflect all the requirements stated in the I in i <br /> Department of Transoortation Regulations for State O erating AssistancP <br /> to Downstate Areas (Title 92, Chapter I, Subchapter h, Part 653). <br /> Please note the following changes from previous years: <br /> Form OP-2 now requires a description of your agency's territorial <br /> boundaries, service area, eligible s�rvice extensions, and ineligible service <br /> extensions to help us further understand the services your agency <br /> provides. <br /> Forms OP-6E. OP-6F. and OP-6G have been revised and requires the <br /> reporting of any significant changes to the inventory from the inventory <br /> filed with the Department in last year's application, including vehicle <br /> dispositions. Please note that the Department will not approve any <br /> applicafion until a completed inventory in the Department's prescribed <br /> format has been received. <br /> Form OP-9 has been revised and requires a change in how paratransit <br /> service operating data is reported. This change will result in consistent <br /> statisticat reporting between the OP-6 and OP-9 reports. <br /> The OP-10 Forms have been revised to reflect the FY00 increased <br /> funding percentage of 53%. <br /> The Schedule of Revenue and Exnense required to be included in your <br /> agency's FY00 independent single audibfinancial statements is being <br /> transmitted at this time as Pages 48 and 49 of the application. <br /> Lastly, other minor changes have been made to individual pages of the <br /> application, and any grantee that currently has the application on � <br /> computer should review and update each page accordingly. <br /> The statutory application filing deadline is April I, 1999.
