' ` Attachment#4 <br /> " � May 7, 1999 <br /> Page 6 of 6 <br /> DRUG FREE WORKPLACE CERTIFICATION <br /> Pursuant to the applicable regulations contained in the Drug Free Workplace Act, <br /> which requires that no contractor shall be considered for the purposes of being awarded <br /> a contract or agreement for the procurement of property or services, unless that <br /> contractor has certified to the State of Illinois that it will provide a drug free workplace. <br /> False certification or violation of the certification may result in sanctions including, but <br /> not limited to, suspension of contract payments, termination of contract, an debarment <br /> of contracting opportunities with the State for at least one (1) year, but not more than <br /> five (5) years. <br /> The Bidder/Contractor by submittal of a Bid, hereby certifies that is in compliance <br /> with the regulations and policy implementation of the Drug Free Workplace Act. <br /> To the extent that the Bidder/Contractor, or any subcontractor, or their <br /> employees, perform a safety sensitive function related to and under this Project, the <br /> Bidder/Contractor agrees to comply with and assures the compliance of each affected <br /> subcontractor and their employees, with 49 USC, Subsection 5331, and FTA <br /> Regulations, "Prevention of Prohibited Drug Us in Transit Operations," 49 CFR, Part <br /> 653. <br /> Signature <br /> Harrv M.
