•r,* .t <br /> AGREEMENT <br /> For and in consideration of the promises, covenants, agreements and conditions <br /> hereinafter set forth to be performed, kept and met by the respective parties hereto, this <br /> Agzeement is made by and between THE CITY OF DECATLJR, ILLINOIS, a Municipal <br /> Corporation, hereinafter referred to as City, and DECATUR TAXI, INCORPOR.ATED, an <br /> Illinois Corporation, authorized to and doing business in the City of Decatur, hereinafter referred <br /> to as the "Cab Company",as follows: <br /> 1. This Agreement shall become and be effective as of Apri16, 1999, and shall continue <br /> in effect until Apri130, 2000; thereafter, the Agreement may continue in effect from year to year, <br /> or until one party or the other shall give notice of its intention and desire to ternunate the same, <br /> not less than ninety (90) days prior to the first day of May, in the year in which it is sought to <br /> cause sa.id termination. <br /> 2. Commencing with the first day of the term of this Agreement and continuing through <br /> the duration thereof, the Cab Company agrees to furnish taxicab transportation for those persons <br /> who are eligible participants in the City's Subsidized Taxi-Cab Program for the Disabled. <br /> 3. The Cab Company shall provide transportation services adequate to meet the needs of <br /> the Decatur Subsidized Taxi-Cab Program for the Disabled, hereinafter referred to as "Program", <br /> and Cab Company shall operate said Program in accordance with guidelines issued by the City, <br /> which shall include, but not be limited to, the following conditions: <br /> A. Program Requirements: <br /> 1. The Cab Company will only transport Program participants who <br /> live within the City limits of Decatur, Illinois. • <br /> - 1 - <br />
