.r� . <br /> 6. Cab Ride forms shall be completely filled out and signed by <br /> both the passenger and the driver before reimbursement will be made. <br /> 7. The City shall pay the voucher within 35 days after it is <br /> received from the Cab Company. <br /> 4. The City shall have the responsibility for establis�ing the guidelines for the Subsidized <br /> T�i-Cab Program. ' <br /> 5. The City shall register Program participants, detemune eligibility, and furnish proper <br /> identification to eligible participants. <br /> 6. The City shall provide adequate supplies of cab ride forms to individual participants, <br /> social agencies, the Cab Company, and other interested parties. <br /> 7. The City shall promote the Program via radio, newspaper, informational brochures, <br /> and various other media as deemed necessary. <br /> 8. The City shall determine the cash faze to be paid by Program participants and reserves <br /> the right to change that rate. <br /> 9. The Cab Company shall be excused for failure to perform services under this <br /> Agreement if said service is prevented by reason of Acts of God, strikes, labor disputes or other <br /> occurrences over which the Cab Company has no control. <br /> 10. If, during the course of this Agreement, the Cab Company fails or refuses to provide <br /> transportation services for the Program for reasons other than those specified above, the City <br /> may, at its option, assess the Cab Company an amount not to exceed $100.00 per day as damages <br /> from the date of discontinuation of service to the date that service is reinstituted. <br /> 11. This Agreement may be modified periodically by mutual agreement of both parties in <br /> order to meet the changing transit needs of Decatur or to create greater operating efficiency. • <br /> - 4 - <br />
