t <br /> � _ <br />� SUBRECIPIENT AGREEMENT <br /> CDBG PROGRAM YEAR 1999 <br /> THIS AGREEMENT, entered this 23rd d�y of February , 1999 by and between the City of <br /> Decatur, Illinois (herein called the "City") and United Community of Shalom (herein called the <br /> "Subrecipient"). <br /> WHEREAS, the Grantee has applied for and received funds from the United States Government <br /> under Title I of the Housing and Community Development Act of 1974, Public Law 93-383; and <br /> WHEREAS,the City wishes to engage the Subrecipient to assist the City in utilizing such funds; <br /> NOW, THEREFORE, i�s agreed between the parties hereto that; <br /> 1. SCOPE OF SERVICE <br /> A. Activities <br /> The subrecipient will be responsible for the Fiscal Year 1999-2000 for the <br /> Old Kings Orchard Community Reorganizations Project. <br /> Program Delivery <br /> Activity: <br /> Benefit to low/moderate income families. <br /> General Administration <br /> B. Levels of Accom�lishment <br /> In addition to the normal administrative services required as part of this Agreement, the <br /> Subrecipient agrees to provide the following levels of program services: <br /> Activi Units per Month Total Units <br /> Recruit neighbors as liaisons N/A N/A <br /> to make referrals, link to <br /> community agencies, interview <br /> and assist in training to address <br /> livability and safety issues <br /> -1- <br />
