ADDITIONAL AND/OR EMERGENCY INSPECTIOWSURVEYS <br /> We are prepared to offer our services to the City of Decatur in the inspection and <br /> evaluation of their sanitary and storm sewer infrastructure by dang whatever it takes to <br /> provide a quality service in a professional, safe and cost-effective manner. We have and <br /> will stay focused on the City of Decatur engineering needs; emphasizing long-term, <br /> mutually beneficial relationships based on trust and respect. <br /> 1. T.V. InspectioNSurveys <br /> 2. Proper documentation and preparation of all field data. <br /> 3. Evaluation of data and recommendations for repair of problem areas. <br /> 4. Field engineering surveys for the location of lost or buried manholes including <br /> horizontal and vertical locations of relocated sewer lines of which there are no <br /> records available. <br /> 5. Emergency work is offered on a 24 hour per day, 7 days per week, with no more <br /> than a two-hour response time. <br /> D. ASSUMPTIONS/CONDITIONS <br /> This Agreement is subject to the following assumptions/conditions: <br /> SCHEDULE <br /> It is anticipated that the T.V. Inspection survey will take place during the months of <br /> October, 1998 through January, 1999 for the estimated 60,000 L.F.with the final report <br /> following 15 days after completion. Any additional footage to be televised or smoke <br /> testing, manhole evaluations and other services we are asked to pertorm will be <br /> completed in a timely manner as agreed to by the City Engineer. <br /> E. ACCEPTANCE OF CONDITIONS STATEMENT <br /> Tele Scan, Inc. hereby�rm that they are in full acceptance of all conditions and <br /> requirements contained in the RFQ/RFP dated August 20, 1998 and are prepared to <br /> execute an agreement with the City of Decatur and perform these services accordingly. <br /> 1. The work shall be performed in compliance with applicable OSHA requirements. <br /> 2. All insurance requirements shall be met. <br /> Tele Scan, Inc. have no employees who are spouses or children of City employees or the <br /> spouse of a City employee's child. <br /> F. SUBCONTRACTORS <br /> Tele Scan, Inc. plans to perform all the services required in this proposal with the <br /> possible exception of sewer cleaning. <br /> It is our understanding that most of the cleaning will be performed by city emptoyees <br /> when possible. <br /> In the event we are asked to perform these services, it will be a combined effort between <br /> Tele Scan, Inc. and Bodine Services, Inc., of Decatur, Illinas for heavy cleaning at an <br /> agreed upon rate with the City Engineer before the work is performed. <br /> 4 <br />
