. . <br /> r� � <br /> , .� d <br /> . . /�' �'i'�i <br /> AGREEMENT <br /> For and in consideration of the promises, covenants, agreements and conditions <br /> hereinafter set forth to be performed, kept and met by the respective parties hereto, this <br /> Agreement is made by and between THE CITY OF DECATUR, ILLINOIS, a Municipal <br /> Corporation, hereinafter referred to as CITY, and CAREER SERVICES ITNLIMITED, INC., <br /> an Illinois Corporation, authorized to and doing business in the City of Decatur, hereinafter <br /> referred to as CONTRACTOR, as follows: <br /> l. This Agreement shall become and be effective as of September 16, 1996 and shall �'�, <br /> continue in effect until September 16, 1999, or until the CITY or CONTRACTOR <br /> shall give notice of its intention and desire to terminate or renegotiate the terms of <br /> the same not less than ninety (90) days prior to September 16th in the year in <br /> which it is sought to cause said termination or renegotiation. <br /> 2. Commencing with the first day of the term of this Agreement and continuing <br /> through the duration thereof, the CITY agrees to provide the vehicle, fuel and oil, <br /> maintenance of vehicle, and vehicle insurance and CONTRACTOR agrees to <br /> provide the personnel necessary to operate the Specialized Van Transportation <br /> program, hereinafter referred to as the Program, for those disabled persons who <br /> are eligible participants. <br /> 3. CONTRACTOR shall provide transportation services adequate to meet the needs <br /> of the Program and CONTRACTOR shall operate said Program in accordance <br /> with guidelines issued by the City, which shall include, but not be limited to, the <br /> following conditions: <br /> A. Program Requirements: <br /> 1. CONTRACTOR will only transport Program participants who are <br /> residents of the City of Decatur, Illinois and whose trip origin and <br /> destination are within the City limits. <br /> 2. Rides shall be scheduled a day in advance to assist in scheduling <br /> and dispatchin�. To ride, all participants will call the dispatcher at <br /> the Decatur Public Transit System (DPTS)the day before they <br />
