Duties Of Corporate Health Services <br /> 1.1 OCCUPATIONAL HEALTH SERVICES agrees to the following: <br /> 1. OCCUPATIONAL HEALTH SERVICES will communicate with THE CLIENT within 24 hours <br /> injury treatment and management which includes:. <br /> A. the results of all physician findings <br /> B. the current diagnosis '� <br /> C. the current work restrictions <br /> D. the treatment plan <br /> E. prognosis <br /> F. any future appointments <br /> This communication is only available on regular business days. (Monday through Friday, 7:00 <br /> A.M. - 6:00 P.M.) <br /> 2. OCCUPATIONAL HEALTH SERVICES agrees to offer THE CLIENT management and injury <br /> tracking for all injuries initially treated at OCCUPATIONAL HEALTH SERVICES. This case <br /> management will continue throughout the recovery process and until the employee is given a full <br /> release to unrestricted duty. <br /> 3. OCCUPATIONAL HEALTH SERVICES agrees to assist THE CLIENT in controlling the cost of <br /> its work related injuries through case management, injury tracking, aggressive treatment, <br /> knowledge of essential job functions and communication. <br /> 1.2 Rehabilitative Services including an appropriate combination of modalities and work <br /> conditioning: <br /> A. functional, objective capacity evaluations: <br /> B. B-200, objective evaluations <br /> C. work conditioning <br />
