SUBRECIPIENT AGREEMENT <br /> THIS AGREEMENT, entered this 25th day of June, 1996 by and between the City of Decatur, <br /> Illinois (herein called the "City") and Macon County Health Department (herein called the <br /> "Subrecipient"). <br /> WHEREAS, the Grantee has applied for and received funds from the United States Government <br /> under Title I of the Housing and Community Development Act of 1974, Public Law 93-383; and <br /> WHEREAS,the City wishes to engage the Subrecipient to assist the City in utilizing such funds; <br /> NOW, THEREFORE, it is agreed between the parties hereto that; . <br /> 1. SCOPE OF SERVICE <br /> A. Activities <br /> Purchase of an lead analyzer. <br /> Program Delivery <br /> Activity: To complete home investigations/lead inspections. Identify lead paint hazards. <br /> General Administration <br /> None <br /> B. v <br /> In addition to the normal administrative services required as part of this Agreement, the <br /> Subrecipient agrees to provide the following levels of program services: <br /> Activitv Units ner Month Total Units <br /> C. �ta� <br /> Jerry Andrews,Administrator <br /> D. Performance Monitorin� <br /> The City will monitor the performance of the Subrecipient against goals and <br /> performance standards required herein. Substandard performance as determined by the City will <br /> constitute non-compliance with this agreement. If action to correct such substandard <br /> performance is not taken by the Subrecipient within a reasonable period of time as specified by <br /> the City, then, after being notified by the City, contract suspension or termination procedures will <br /> be initiated. <br />
