� t . . <br /> AGREEMENT <br /> :, <br /> �, �� ;; , <br /> AGREEMENT made this � ' day of � i,( ' 1995, by and between <br /> R <br /> r <br /> the City of Decatur, Illinois, ("City"), a municipal corporation, and Decatur-Macon County Senior <br /> Center, Inc., an Illinois not-for-profit corporation, ("Center"). <br /> WHEREAS, for many years the City has performed certain functions and services through <br /> its Office on Aging for the benefit of senior citizens of Decatur, which functions and services axe <br /> more fully described herein; and, <br /> WHEREAS, the City Council now finds that such functions and services as are outlined in <br /> this Agreement can be performed more economically and efficiently by the Center, working under <br /> contract with the City; and, <br /> WHEREAS, the Center acknowledges that said functions and services are important to <br /> the City and its citizens and agrees to perform same as per the provisions hereof. <br /> Now, Therefore, for and in consideration of the following terms and conditions, the City <br /> and the Center agree as follows: <br /> 1. DEFINITIONS AND RESPONSIBILITIES. As used in this Agreement, unless a <br /> contrary intent is clearly evident, the following words and phrases shall have the meaning ascribed <br /> to each herein: <br /> Senior Citizen - a person fifty-five (55) years of age or <br /> older. <br /> Information and Assistance - a service whereby the service <br /> provider, upon request, provides a requesting senior citizen, or care <br /> giver, with information regarding various substantive services <br /> available to the requester. Such requests can be made by telephone, <br /> by mail, or in person. Examples of available substantive services <br /> would include, in addition to those specified herein, and without <br /> limitation, financial counseling, medical services, and nutritional and <br />
