Weaver, Community Development Director • <br /> SUB7ECT: Resolution Entering Into Agreement For Senior Citizen ervices <br /> Enclosed please find the resolution and agreement covering senior citizen services for the period <br /> September 1, 1995 through August 31, 1996. City staff developed this agreement consistent with <br /> the provisions of the request for proposals and in cooperation with the Senior Center staff. We <br /> have met with senior center staff and they have concuned with the provisions of the agreement. <br /> You will note the agreement provides for numeric and performance thresholds as well as monthly <br /> reporting requirements. The agreement is renewable for two additional 12 month periods. <br /> To ensure a smooth transition, city staffwill temporarily work at the senior center facility. Upon <br /> agreement approval, arrangements will be made to move furniture and equipment to the senior <br /> center for the month of September. This 30 day period will assure a smooth transition of service <br /> between the city and senior center. City responsibility for senior citizen Information and <br /> Assistance and Outreach will cease September 30, 1995. <br /> Staff will be present to answer any questions. <br />
