• A reement for Ins ection Senrices <br /> a P --- <br /> Cit,y of Decatur <br /> Public Transit Facilitv <br /> Januarx 20. 1995 <br /> This is an agreement between the City of Decatur (Owner) and Blank, Wesselink, Cook & <br /> Associates, Incorporated (BWC). This agreement is necessary since the Owner is seeking to <br /> provide more extensive on-site representation to enhance the quality control of the project. <br /> The project is the construction of the new City of Decatur Mass Transit Facility. <br /> BWC is hereby proposing to provide the additional on-site representation as sought by the ' <br /> Owner. The on-site representative site visits will average finro (2) days per week at eight (8) <br /> hours per day. It is estimated that there are 23 weeks remaining in the construction period. <br /> This would bring the estimated total time for inspection services to 368 hours. It should be <br /> understood that there may be times when the construction schedule requires full-time <br /> i^s�ectior as ��;eE! as times ���er� th� �eEd �oulu be less. <br /> The compensation for the on-site project representative(s) will be in accordance with Exhibit A � <br /> of the Agreement. The compensation of the representative(s) shall be as follows: <br /> Wage Rate = $19.37 per hour <br /> plus 35.00% DPE = 6.78 per hour <br /> Subtotal = $ 26.15 per hour <br /> 2.04 Multiplier * 2.04 <br /> Total Hourly Rate = $53.34 per hour <br /> Estimated total compensation = 368 * $53.34 = $19,629.12 <br /> For Blank, Wesselink, Cook & A sociates, Inc. ,.� <br /> � ,, <br /> Har M.
