Such t_ans ortation shall be coniined to Macon Count �':�;` _ P y, or to the service :_'��;,;; area o� the local taxi-cab company, whichever is more restrict:ve. .-.�.:,. 3. Transportation wi11 be provided to participants in the Medicare Alzheimer's ; Disease Demonstration project and to their escorts. Fiowever, the taxi- cab operators are not trained medical or emergeac9 technicians, therefore, participants requiring any medical assistance cannot be traasported. 4. The C�ty shall biil the Car1e Cl�nic Association, P.C. , 602 West lin- vers'_t� Avenue, Urbana, Illinois 61801 for services rendered under this Agreement on a montaiy basis. 5. The rates to be charged bv the City wi11 be the standard rates of the taxi-cab companv. Those rates a�e S2.00 initial charge plus ten cents for eac11 one-tenth (1/10) mi1? thereafter, with a minimum fare of S3 .00. Extra persons w�ll be charged S.50 each. 6. If recu��ed, the Citv shall issue identification cards to partic�pants ' in the Medicare Alzheimer's Disease Demonstration project who cri?i be ' using the transportat=on services . 7. The Citv shal?, if required, maintain records of the times and dates that the transporta��on services are utilized, tne orig_n and desti- nation of each t�ip. the purpose of the trip and the length anc cost of each trip. 8. If anv participants in the Medicare Alzheimer Disease Demonstration proiect are wheelchair bound, the City will transport them in its lif�-equ�pped van. 9• Transportation of wheeZchair bound persons in the Citq's special van shal? be restric�ed to the Cit� limits and shail be in accordance with the guidelines for the program 10. The cha�ge for use of the van shail be seventy-five cents per one- way trip. The same fee wiil be charged for escorts wno accompaay partici�ants in the project.
