Weaver, Director of Community Development ` <br /> Ellen Snowdon, Administrator, Office on Aging <br /> SUBJECT: Medicare Alzheimer's Disease Demonstration Program <br /> The attached contract covers renewal of Information and Referral and <br /> Outreach services to be provided by the Office on Aging and <br /> Transportation services by the Decatur Public Transit System to <br /> clients of the Carle Clinic Medicare Alzheimer's Disease <br /> Demonstration Program_ The period of renewal is November l, 1993 to <br /> October 31, 1994_ <br /> Services covered in this contract are normally provided by the Office <br /> on Aging and the Decatur Public Transit System. There will be no ' <br /> additional services or expenses incurred. The contract allows the ', <br /> Of£ice on Aging and DPTS to be reimbursed the cost of services as I <br /> stated in Exhibit A. � <br /> This program began as a three year demonstration project in 1989 to �I <br /> fund care and assistance from local agencies for Alzheimer's victims � <br /> and their caregivers. The Health Care Financing Administration <br /> (HCFA) is evaluating the services needed, the impact of <br /> community-based services on the individuals and their families, and <br /> the relative cost of community-based services as compared to <br /> institutional services currently covered by Medicare. <br /> HCFA also recognizes the role and the special needs of the caregiver_ <br /> This program includes education, training and emotional support for <br /> the caregiver to minimize the stress that comes from providing <br /> continuous care for an Alzheimer's victim. <br />
