Provider shall at ali times cooperate with and p�ovide to Carie Clinic <br /> any and att inforcnation requested by Carie Ciinic wfiich it deems necessary to compiete <br /> reports requesied by HCFA and/or suppfy infocmation required by HCFA. <br /> SECTION Vlll - ACCOUNTiNG PRfNCiPtES <br /> 8.1 For purpos2s of the Demonstration, Provider shall be subject to the same <br /> accounting and reimbursement requir�ments as Carfe Ctinic, namefy accrua( accounting and <br /> retention of books and records for fve years. <br /> SECTION IX - QUALITY ASSURANCE AND UTtUZAT10N CONTROL PROGRAMS <br /> 9.1 Provider shail, at ail times, cooperate with the Medicai Director of Carie <br /> Clinic, or other designated individuats, in tfie conduct of such Qua(ity Assurance Programs and <br /> Utilization Convot Programs as may be estab(ished by Carie Clinic and in effect from time-to- <br /> time during the term of this Agreement. <br /> 9.2 Provider shall, at all times, follow such pre-certification p�ocedures as may <br /> be esrabiished by Carte Ctinic and in effect from time-to-time during the term of this <br /> Agresment. Fail�re to cooperate with such pre-certification procecfures may resutt in the loss <br /> of reimbu�sement to Provider. <br /> 9.3 Provider certifies that al( nurses and other anciilary and paramedic <br /> personne! empiayed by Provider, or retained by Provider under convact, eit}�er written or oral, <br /> to provide Demonstration Service to Patients are (icensed, certi�ied o� registered by all <br /> appiicable federal, state and/or local Iicensing, certirying or registering badies, as required to <br /> perform their duties and that Provider and eacf�of such personnel, wiil maintain said ticenses, <br /> certin"cations and/or registrations in good standing at ail times during the term of this <br /> Agreement. Faiiurs of Provider to compiy with the provisions of this paragraph 9.3 shall be <br /> deemed to be a basis for termination of this Agresment "for cause" as set forth hereafter in <br /> paragraph 13.3. <br /> T " <br /> � � 4 <br />
