� � � MEDICARE ALZHEIMER'S <br /> DISEASE DEMONSTRATION <br /> A Health Care Financing Administration demonstrarion <br /> coordinated by Carle Clinic Association <br /> 2009 Round Barn Road, Champaign, IL 81821 <br /> (2171 351-1738 <br /> November 9, 1992 <br /> James Bacon <br /> City Manager <br /> City of Decatur <br /> #1 Civic Center Plaza <br /> Decatur, IL <br /> Dear Mr. Bacon: <br /> Enclosed are two (2) copies of the 1992-1993 Service Provider Agreement for <br /> the Medicare Alzheimer's Disease Demonstration. Please sign both contracts <br /> and return one to us, keeping the other for your files. Ellen Snowden and <br /> Robert Reed have stated that Exhibit A will have no changes in services or price <br /> increases. <br /> In light of the possibility that the demonstration may not be extended past the <br /> scheduled May 14, 1993, termination date, we are calling your attention to <br /> Section 13.4 of the Contract which requires "at least ninety (90) days notice <br />' in writing of the intended termination date." Barring an extension, <br /> demonstration services will be terminated April 30, 1993. <br /> There are bills containing extension language now being considered by various <br /> congressional committees. There is a possibility that if the demonstration is not <br /> extended this year that it might be extended in the early Spring of 1993. <br /> We plan to keep clients, families and providers advised of any news and assure <br /> you that any notice of termination will be given as promptly as possible. We <br /> will be working with clients, families and providers to develop discharge plans <br /> and ask that you encourage client/caregivers to contact their Case Manager <br /> with any questions or concerns.
