Social work services include counseling services designed to promote <br /> rna�dmum levels of physical and mental func�ioning by providing <br /> psycholo�ical suppor to deal with particuIar personal and social_�roblems. <br /> 4. Parcicipant activity services inelude planned recreational and social <br /> acrivities suited to the needs of participants and designed to encourage <br /> pnysical and mental exercise and stimulate social inte:action. <br /> 5. T:ansportarion services include a participant's transportarion between <br /> his/her home and the program faciiity. Transportation may be provided <br /> directly or by agreement or contract with another communiry agency or <br /> local specialized transportarion service. <br /> Transportation services must be able to accommodate participanu who <br /> are wheelchair bound.
