r <br /> records which are necessary to verify the nature and extent of costs of services furnished <br /> under this Agreement. Provider shall at all times cooperate with and provide to Carle Clinic <br /> any and all information requested by Carle Clinic which it deems necessary to complete <br /> reports requested by HCFA and/or supply information required by HCFA. <br /> SECTfON VIII - ACCOUNTING PRINCIPLES <br /> 8.1 For purposes of the Demonstration, Provider shall be subject to the same <br /> accounting and reimbursement requirements as Carle Clinic, namefy accrual accounting and <br /> retention of books and records for five years. <br /> SECTION IX - QUALITY ASSURANCE AND UTILIZATION CONTROL PROGRAMS <br /> 9.1 Provider shall, at all times, cooperate with the Medical Director of Carle <br /> Clinic, or other designated individuals, in the conduct of such Quality Assurance Programs and <br /> Utilization Control Programs as may be established by Carle Clinic and in effect from time-to- <br /> time during the term of this Agreement. <br /> 9.2 Provider shall, at all times, follow such pre-certification procedures as may <br /> be established by Carle Clinic and in effect from time-to-time during the term of this <br /> Agreement. Failure to cooperate with such pre-certification procedures may resul`_in the loss <br /> of reimbursement to Provider. <br /> 9.3 Provider certifies that all nurses and other ancillary and paramedic <br /> personnel employed by Provider, or retained by Provider under contract, either written or oral, <br /> to provide Demonstration Service to Patients are licensed, certified or registered by all <br /> applicable federal, state and/or local licensing, certifying or registering bodies, as required to <br /> perform their duties and that Provider and each of such personnel, will maintain said licenses, <br /> certifications and/or registrations in good standing at all times during the term of this <br /> Agreement. Failure of Provider to comply with the provisions of this paragraph 9.3 shall be <br /> deemed to be a basis for termination of this Agreement "for cause" as set forth hereafter in <br /> paragraph 13.3. <br /> 4 <br />
