' EXHIBIT A Section 2 <br /> 1. The City of Decatur, Illinois, thru its contract with the local taxi- <br /> cab company, shall arrange for transportation af participants in the <br /> Medicare Alzheimer's Disease Demonstration project <br /> 2. Such transportation shall be confined to Macon County, or to the service <br /> area of the local taxi-cab company, whichever is more restrictive. <br /> 3. Transportation will be provided to participants in the Medicare Alzheimer's <br /> Disease Demonstration project and to their escorts. However, the taxi- <br /> cab operators are not trained medical or emergency technicians, therefore, <br /> participants requiring any medical assistance cannot be transport�d. <br /> 4. The City shall bill the Carle Clinic Association, P.C. , 602 West Un- <br /> versity Avenue, Urbana, Illinois 61801 for services rendered under this <br /> Agreement on a monthly basis. <br /> 5. The rates to be charged by the City will be the standard rates of the <br /> taxi-cab company. Those rates a�e $2.00 initial charge plus ten cents <br /> for eac�l one-tenth (1/10) milP thereafter, with a minimum fare of <br /> $3.00. Extra persons will be charged $.50 each. <br /> 6. If required, the City shall issue identification cards to participants <br /> in the Medicare Alzheimer's Disease Demonstration project who will be <br /> using the transportation services. <br /> 7. The City shall, if required, maintain records of the times and dates <br /> that the transportation services are utilized, the origin and desti- <br /> nation of each trip, the purpose of the trip and the length and cost <br /> of each trip. <br /> 8. If any participants in the Medicare Alzheimer Disease Demonstration <br /> project are wheelchair bound, the City w�ll transport them in its <br /> lift-equipped van. <br /> 9. Transportation of wheelchair bound persons in the City's special <br /> van shall be restricted to the City limits and shall be in accordance <br /> with the guidelines for the program <br /> 10. The charge for use of the van shall be seventy-five cents per one- <br /> way trip. The same fee will be charged for escorts who accompany <br /> participants in the project. <br />
