. . * <br /> PROPOSED ICF/DD-15 <br /> E. John St. <br /> Decatur, Zllinois <br /> The proposed project is to be located in the old Murheids <br /> Nursing Home. The buildinq which consists of approximately <br /> 10,000 square feet, will be entirely renovated and separated into <br /> two sections. Each section will consist of approximately 5000 <br /> square feet. The East section will be used as the licensed <br /> ZCF/DD-15. The West section will be used for our officss and/or <br /> therapy recreation and storage areas. <br /> The ICF/DD portion will consist of eight semi-private <br /> bedrooms, two living rooms, two dining rooms, office, kitchen, <br /> four full baths, 2 half-baths for visitors and activity room. <br /> For those who are not familiar with an ICF/DD-15, it can <br /> best be described as follows; a facility licensed by the Illinois <br /> Department of Public Health, certified for payment under the <br /> Medicaid program, for Developmentally Disabled Adults over the <br /> age of eighteen years. <br /> It is the intent of the facility to serve Macon County <br /> residents who are now residing at home with parents, in room and <br /> board f acilities and long-term care nursing facilities who <br /> require supervision and training appropriate to the services <br /> provided by our staff. Each resident must be able to ambulate <br /> independently, self-administer their own medications, egress <br /> independently in cases of emergencies and must "not' require <br /> professional nursing service. Each resident must also be <br /> involved in day programs outside of the facility, five days a <br /> week, five hours each day. Most residents will attend Macon <br /> County Rehab, Progress Resources Center or Decatur Public Schooi. <br /> The sponsor of the proposed project, Alpha-Omega Consul- <br /> tants, operates five other similar facilities. Their locations <br /> are Decatur, Charleston, Shelbyville, Taylorville and Loving- <br /> ton. They also have projects under consideration or construction <br /> in Lincoln, Olney and Vandalia, Illinois. As in all operations, <br /> Alpha-Omega Consultants coordinate the residents referrals <br /> through the county case-coordination unit, which in Macon County <br /> is Progress Resources Center. <br />
