<br /> THIS AGREEMENT, made and entered into this �� � "`` � day of �� `_'- ' �' _,

<br /> A.D. 19��., by and between the State of Illinois, acting by and through its Department�f Public Works and
<br /> Buildings, hereinafter referred to as the "Department" and
<br /> a municipal corporation organized and existing under and by virtue of the laws of the State of Illinois, hereinafter
<br /> referred to as the "Corporation."
<br /> WITNESSETH, that for and in consideration of the covenants hereinafter mentioned, the Corporation agrees
<br /> to maintain, for a period ending not later than Jm�e �# , 193�, in a manner satisfactory
<br /> to the Department, portions of certain streets or thoroughfares being used as extensions or parts of State High-
<br /> ways and lying within the limits of said Corporation, such maintenance to include all necessary repairs, cleaning
<br /> and snow removal, the portions of the streets or thoroughfares to be maintained being described in the ta.bulation
<br /> attached hereto and made a part hereof.
<br /> In consideration of the satisfactory maintenance of the herein described streets or thoroughfares by the Cor-
<br /> portation, the Department will pay the said Corporation the actual cost of said maintenance subject to the follow-
<br /> ing conditions:
<br /> The amount to be paid by the Department for repairs, snow removal and all other items of maintenance ex-
<br /> pense, except street cleaning, shall not exceed the sum of

<br /> The amount to be paid by the Department for street clea,ning, shall not exceed the sum of

<br /> The total amount to be paid by the Department to the Corporation under the terms of this agreement, includ-
<br /> ing the cost of repairs, snow removal, street cleaning, the City's contribution to the Illinois Municipal Retirement
<br /> and Benefit Fund, and all other maintenance expense shall in no event exceed the sum of

<br /> for the yea,r ending 1� , 19�. It is understood and agreed that this agreement will

<br /> and amounts mentioned herein unless the Department or the corporation, when in its judgment a Ghan�e in the
<br /> agreement or amount is necessary, gives written notice to the other party not less than thirty (3�) days prior to

<br /> All materials and each part or detail of the work shall be subject to inspection by the Department and the
<br /> right is reserved to reject any materials or work found to be unsa.tisfactory.
<br /> It is hereby understood and agreed that except in cases of extreme emergency, the Corporation will give the
<br /> Department at least ten (10) da.ys notice in writing before beginning work on any repairs estimated to cost in excess

<br /> The Corporation shall not undertake such repairs at the expense of the Department without first securing the
<br /> approval of the Department.
<br /> On March 31, June 30, September 30 and December 31 of each yeax, the Corporation shall submit to the De-
<br /> partment itemized invoices in quadruplicate covering the actual cost incurred by the Corporation in th�maintenance
<br /> of said streets or thoroughfaxes during the preceding period. Upon receipt of and approval of said invoices by the
<br /> Department, the said Department will pay to the Corporation the amount of such invoices.
<br /> It is further understood and agreed that tlie Depart�nent, when in its judgment it is ea�pedient to do so,and at
<br /> its diseretion, shall�have the right to terminat�e this agreement by giving�written notice to the Corporatiorn�of�not
<br /> less than thirty (30) da,ys in advance of the date of such termination.
