CITY OF DECATUR <br /> GENERAL CONDITIONS <br /> GC-1 PERMITS <br /> It is the Contractor's responsibility to obtain all necessary permits. Any costs incurred in <br /> obtaining these permits are to be included with the related pay items in the contract. Permit fees <br /> will be waived for those permits issued by the City of Decatur. <br /> GC-2 PHOTOGRAPHS OF EXISTING CONDITIONS <br /> The Contractor shall provide VHS or DVD video of existing facilities along the route or <br /> area of all construction,prior to the start of such construction. Construction of this facility will <br /> generally be along street right-of-way containing trees,bushes, fences, driveways and similar <br /> items. Special attention shall be given to coverage of the entire easement and immediate <br /> adjacent areas which might be disturbed during construction. <br /> The videos shall be adequate to serve as a basis for comparison in determining whether <br /> the terms of the specifications with respect to replacements,restoration, and/or preservation of <br /> existing surfaces have been complied with. In areas with on-street parking, it will be necessary <br /> to video tape at a time when a majority of parking spaces are clear of vehicles so that existing <br /> facilities, including utilities, are unobstructed from view. One print of the video taken shall be <br /> given to the Engineer for his files. The cost for this item shall be included in the respective <br /> prices for the various items of work. <br /> GC-3 USE OF FIRE HYDRANTS <br /> Prior to any use of fire hydrants b the Contractor, a permit shall be obtained from the <br /> Y Y � <br /> Water Services Division in accordance with Sections 20 and 23 of Chapter 64 of the City Code. <br /> Water Services Division is located at 2600 North Jasper Street; phone 217-875-5705. The <br /> permit will be issued at no charge to the Contractor. <br /> GC4 UTILITIES <br /> ' Contractor shall contact JULIE (1-800-892-0123)prior to digging to locate underground <br /> utilities.
