R2424 --Or�- RESOLUTION AUT�I(JRIZING AN AGREEMENT BETWEEN THE CITY 4F DECATUR AND ILLINOIS PRIMARY�IEALTH CARE ASSOCIATI4N(IPHCA) WHEREAS, the CITY QF DECATUR, ILLINOIS, an Illinois municipal corporation and body politic {"City"} is a unit af local government pursuant to Article VII, Section 6 of the Illinois Canstitution af 197Q; and, WHEREAS, the City finds that Illinois Primary Health Care Association (IPHCA), is a trade association representing Illinois' community/migrant health centers and as such is committed to the irnprovement of the health issues of inedically underserved populations throughout the State of Illinois; and, WHEREAS, pursuant ta the Grant Agreement, IPHCA will provide funding ta the City in the total of $25,000 to enable Decatur to provide census autreach services to within the City to locate hard to count(HTC)populations and conduct census education and autreach efforts. NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Decatur as follows: Section 1. That the City of Decatur appraves entering into an Agreement with Illinois Primary Health Care Association (IPHCA) for 2020 census support depicted in Exhibit A attached hereta and hereby incorporated by reference. Section 2. That the City Manager af the City af Decatur is hereby autharized to enter into and execute the attached Agreement and any other documents necessary ta effectuate this partnership. Section 3. That the City Manager is also hereby autharized to execute any amendtnent that increase the amount af grant funds available to the City, which does not otherwise change the terms of the agreement.
