RESOLUTION NO. 2015-49 <br /> RESOLUTION AUTHORIZING AGREEMENT <br /> JL HUBBARD dba SBSi SEMONIS <br /> WHEREAS, the City of Decatur is in need of the professional brokerage and employee <br /> benefit consulting service to secure group health benefit products and services and to assist in <br /> managing, the City's group health benefit program for the period June 1, 2015, through May 31, <br /> 2017; and <br /> WHEREAS, the benefit consulting firm of JL Hubbard dba SBSi Semonis is prepared to <br /> provide these services the City of Decatur; <br /> BE IT THEREFORE RESOLVED BY THE CITY COUNCIL OF THE CITY OF <br /> DECATUR, ILLINOIS: <br /> Section 1. That the proposal of JL Hubbard dba SBSi Semonis for the provision of <br /> professional brokerage and employee group health benefit consulting services to the City of <br /> Decatur in the annual amount of $40,000 for period of June 1, 2015 through May 31, 2017 be, <br /> and the same is hereby, accepted and approved. <br /> Section 2. That the City Manager be, and he is hereby, authorized and directed to <br /> negotiate, for and on behalf of the City, an agreement with JL Hubbard dba SBSi Semonis for <br /> the above mentioned services in accordance with the proposal for same submitted by said <br /> company. <br /> Section 3. That the City Manager and City Clerk be, and they are hereby, authorized <br /> and directed to execute the attached letter of understanding between the City of Decatur and JL <br /> Hubbard dba SBSi Semonis for the above-mentioned services. <br /> Section 4. That the City Manager and City Clerk be, and they are hereby, authorized <br /> and directed to execute the attached business associate agreement between the City of <br /> Decatur and JL Hubbard dba SBSi Semonis in connection with the above-mentioned services. <br /> PRESENTED AND ADOPTED this_20th day of April 2015. <br /> C VLL f.
