Addendum to Call One's Customer Service Agreement, <br /> Terms and Conditions <br /> City of Decatur <br /> Paragraph 11 (Jurisdiction/Collection Costs)of the Terms and Conditions of RFQ#3592229086, CSA <br /> #34765, CSA#34767, and CSA#34779 shall be amended as follows. <br /> Paragraph 11 (Jurisdiction/Collection Costs): <br /> Notwithstanding the terms of this paragraph, any action or proceeding arising out of or related to this <br /> Agreement, Call One's tariffs or the Services may be commenced in Macon County, Illinois or Federal <br /> court or agency of competent jurisdiction in the State of Illinois.The Parties submit and expressly <br /> consent to the jurisdiction of such court or agency and expressly waive any right to a trial byjury.
