AD-1-2 SPECIAL PROVISIONS (CONTINUED) <br /> 2.02 Add Special Provision SP-13 <br /> SP-13 GRAND AVENUE CURB,SIDEWALK,AND PIPE UNDERDRAIN <br /> Description: Replacement of the 2-foot-wide section of sidewalk behind <br /> the curb along Grand Avenue may be necessary if the loss of support from the <br /> removai of the curb and gutter causes the sidewalk to fail. Similarly,replacement <br /> of the catch basin which the pipe underdrain ties into may be necessary,if its <br /> condition causes it to fail with the loss of support or from the installation of the <br /> pipe underdrain. The Contractor will be solely responsible for the replacement of <br /> the sidewalk and the catch basin if damage is caused by a lack of care on the part <br /> of the Contractor. <br /> Basis of Payment: The cost of the replacement of the sidewallc will be <br /> paid for at the SQUARE FOOT unit price for PORTLAND CEMENT <br /> CONCRETE SIDEWALK, 5"and the SQUARE FOOT unit price for <br /> SIDEWALK REMOVAL. <br /> The cost of the replacement of the catch basin will be paid for EACH for CATCH <br /> BASIN TO BE RECONSTRUCTED WITH NEW TYPE 3 FRAME AND <br /> GRATE. <br /> AD-1-3 PLANS FOR CITY PRUJECT# 19-08 STREET RESTURATION <br /> 3.41 Replace the following plan sheets, attached to this addendum: <br /> • Page 4 <br /> • Page 6 <br /> • Page 12 <br /> • Page 13 <br /> • Page 17 <br /> The revisions on these pages reflect the addition of quantities for <br /> sidewalk removal and replacement, and catch basin reconstruction <br /> if needed.
