Exhibit A <br /> Cost estimates will be prepared based on initial construction cost to complete the work using <br /> itemized costs. The itemized cost estimates will generally include all significant pay items <br /> necessary to complete the proposed work to include the following: <br /> • Structure pay items to include structure repairs <br /> • Staging &Traffic Control related to the structure <br /> • Profile revisions necessary for structure related issues such as clearances <br /> The scope of work selection is the final step in the bridge analysis process. It requires a <br /> review of the critical factors surrounding the structure and project. These factors can be <br /> separated into the following categories: <br /> 1. Structure Condition and Load Capacity <br /> 2. Geometric Acceptability <br /> 3. Economic Evaluation <br /> 4. Exterior Constraints <br /> Pin & Link Special Inspection—Garfield Avenue Bridge over 22"d Street <br /> This work will include the following: <br /> 1. Perform an arms-length inspection, utilizing visual and ultrasonic testing equipment in <br /> sufficient detail to allow completion of the required IDOT Special Inspection and <br /> Preliminary Bridge Inspection Forms (BBS-SI-1 -Special Inspection & BBS 2760- <br /> Preliminary Pin and Link Inspection Journal). <br /> 2. The scope of this work follows the basic pass-fail criteria for pin inspection utilizing <br /> ultrasonic testing as described in Sections 3.5 &3.6 of the IDOT Structural Services <br /> Manual, but does not cover the Supplemental Pin Inspection procedures as described in <br /> Sections 3.6.1.4.2 and 3.6.1.5 of the same manual. <br /> 3. Coordinate the inspection activity and report filing with the City of Decatur and the Illinois <br /> Department of Transportation. <br /> Special Equipment/Traffic Control Requirement for Bridge Inspections <br /> The following is a summary of anticipated needs. <br /> Garfield Avenue Bridge over 22nd Street (IL 121) (SN 058-6001): AECOM intends to rent a <br /> single man-lift in order to inspect the bottom of the structure. Lanes on 22"d Street will need to <br /> be closed during the inspection and prior to delivery of the man-lift. Having two adjacent lanes <br /> closed would provide a safer working area for the man-lift, put previously that was not able to be <br /> provided— instead, one lane at a time in each direction was closed, and once the inspection <br /> A-5 <br /> Rev 0—12/10/2020 <br />
