Exhibit A <br /> Detailed Scope of Services—City of Decatur <br /> 2021 Bridge Inspection—Structure Number 058-6001 <br /> Perform bridge inspection to be conducted by an engineer with a minimum of <br /> Team Leader certification and conforming to the requirements of the National <br /> Bridge Inspection Standards (NBIS) and the requirements of the Illinois <br /> Department of Transportation. Inspection services will include providing <br /> equipment and qualified staff to perform the following inspections: <br /> January 2021 <br /> • Garfield Avenue Bridge over 22nd Street (IL 121): 058-6001, In-depth <br /> (hands-on) Routine Inspection <br /> • Garfield Avenue Bridge over 22"d Street(IL 121): 058-6001, Pin & Link <br /> Special Inspection <br /> The work will include the preparation of the necessary bridge inspection reports <br /> conforming to IDOT requirements. A report will be prepared as an update to the <br /> Bridge Condition Report(BCR)prepared by AECOM in 2017, detailing <br /> recommended repairs. The report shall be of sufficient scope to serve as a basis <br /> for future design projects. <br /> Bridge Condition Reports <br /> The updated Bridge Condition Reports provide a format for the City to develop and document a <br /> proposed scope of work for the structure. The reports are submitted to the Bureau of Bridges & <br /> Structures (BB&S)for review and approval. <br /> Bridge Inspection Process <br /> The inspections will be completed in accordance with the procedures and terminology outlined <br /> in: the Bridge Inspectors Reference Manual (BIRM), Federal Highway Administration (FHWA <br /> latest edition); the Recording and Coding Guide for the Structure Inventory and Appraisal of the <br /> Nation's Bridges (FHWA latest edition); the Illinois Department of Transportation (IDOT) <br /> Structural Information and Procedures Manual; Section 3.3 of the IDOT Structural Services <br /> Manual; Section II of the IDOT Manual for Bridge Condition Report Procedures & Practices; and <br /> other applicable and approved procedures and methods as noted in the applicable FHWA and <br /> IDOT memoranda. <br /> Bridge inspection activity and report filing will be coordinated with the City of Decatur and the <br /> Illinois Department of Transportation. <br /> Preparation for In-Depth and Special Inspections <br /> Prior to performing the in-depth and special inspections, the following tasks will be <br /> performed: <br /> A-1 <br /> Rev 0—12/10/2020 <br />
