Exhibit A <br /> 1. Review the Routine Inspection and Special Inspection Forms BBS-BIR & BBS-SI-1 which <br /> will be downloaded from the Illinois Department of Transportation web site. <br /> 2. Obtain the Master Structure Report (S-107)for the structures to be inspected from the <br /> Illinois Department of Transportation web site. <br /> 3. Review existing drawings of the structures to be inspected. Note any previous <br /> rehabilitation work. <br /> 4. Prepare sketch plans of the top and bottom of the deck, abutments, piers and other <br /> structural elements as necessary to allow proper documentation of the location and <br /> description of significant distress features. <br /> 5. Prepare a list of equipment needed to complete the inspection. Consider how each <br /> element of the bridge to be inspected will be accessed to determine how special <br /> equipment and traffic control will be utilized. Traffic control needs will be coordinated with <br /> the City of Decatur; City will provide all traffic control, with the exception of a crash truck <br /> behind to be provided by a traffic control Contractor. <br /> In-Depth Bridge Inspections <br /> In-depth bridge inspections will be performed in accordance with Section 3 of the IDOT <br /> Structural Services Manual and Section II of the IDOT Manual for Bridge Condition Report <br /> Procedures & Practices. Work will include the following activities: <br /> 1. Coordinate the delivery of special Ultrasonic Testing (UT) equipment. <br /> 2. Coordinate with City of Decatur on set-up of traffic control, as needed. <br /> 3. Inspect the bottom of the deck. <br /> 4. Inspect the top of the deck along with the parapet/railing. <br /> 5. Inspect transverse and longitudinal deck joints. <br /> 6. Inspect beams and girders. <br /> 7. Inspect bearings. <br /> 8. Inspect abutments. <br /> 9. Inspect piers. <br /> 10. Inspect other items as appropriate: <br /> a. presence and extent of erosion at the site <br /> b. presence, types and condition of utilities on or near the structure <br /> c. condition of the slope protection system <br /> Document Inspection Results <br /> Proper documentation of the inspection results is critical to producing a good Bridge Condition <br /> Report and to determine the appropriate scope of work.
