CITY OF DECATUR <br /> COMMUNITY DEVELOPMENT BLOCK GRANT <br /> CONTRACTURAL SERVICE AGREEMENT <br /> This Community Development Block Grant Agreement (the "AGREEMENT") is made on <br /> this 1st day of November 2021, by and between the City of <br /> Decatur, an Illinois Municipal Corporation (hereinafter "CITY") and Barry G. Maciak, DBA Key <br /> Strategic (hereinafter "CONTRACTOR"). <br /> WHEREAS,the CITY has received funds from the United States Government the U.S. Department <br /> of Housing and Urban Development ("HUD") under Title 24 of the Code of Federal Regulations, <br /> Part 570 and 2 CFR Part 200 of Uniform Administrative Requirements, Cost Principles and Audit <br /> Requirements; and, <br /> WHEREAS, the CITY wishes to engage the CONTRACTOR to assist the CITY in utilizing such funds <br /> in accordance with HUD guidelines and Federal regulations for the development of the <br /> Workforce Development Systems Assessment, Strategy and Implementation Project. <br /> NOW, THEREFORE, it is agreed between the parties hereto that: <br /> I. APPENDICES TO AGREEMENT: All Appendices (A through D) attached to this Agreement <br /> are incorporated and made a part of this Agreement as referenced herein. CONTRACTOR <br /> agrees to abide by and follow all terms and conditions as set forth in this Agreement. <br /> II. WORK TO BE PERFORMED BY CONTRACTOR: In exchange for receiving funds from the <br /> CITY for the Workforce Development Systems Assessment, Strategy and Implementation <br /> Project(hereinafter"Program"),the CONTRACTOR shall be responsible forthe objectives, <br /> tasks and scope of services as outlined in appendix D in a manner satisfactory to the CITY <br /> and consistent with any standards and regulations that are specified in this Agreement as <br /> a condition of providing these funds. <br /> The program will demonstrate a new service and an increase in the level of an existing <br /> service as required under the Housing and Urban Development (HUD) regulations. <br /> CONTRACTOR agrees to expend the funds covered in this Agreement, solely for the <br /> agreed upon activities and in accordance with the conditions outlined in this Agreement <br /> and the Program budget in Appendix B. <br /> III. CERTIFICATION:To the best of its knowledge or belief,the CONTRACTOR certifies that: <br /> A. No federal appropriated funds have been paid or will be paid, by or on behalf of <br /> the undersigned, to any person for influencing or attempting to influence an <br /> officer or employee of any agency, a member of Congress, an officer or employee <br />
