HEALS will track the number of families/individuals that transition from receiving <br /> Crisis Support services to Case Management services and what services are <br /> provided in regards to relocation if that goal was established. <br /> - Crisis Support workers will track the number of times follow up information is <br /> shared, if questions are asked, the number of victim compensation packets they <br /> assist with <br /> Data will be collected regarding the number of individuals that engage in <br /> counseling services <br /> Crisis Response workers will collect observable data regarding parents demeanor <br /> at specific intervals following their crisis (i.e. at the crisis support workers first <br /> interaction with the individual, at 1 week following the crisis, at 1 month following <br /> the crisis) <br /> Service satisfaction surveys will be sent to all who engage at a specific time <br /> frame following their crisis. <br /> Budget Needs <br /> - Currently have funding from United Way, Ballance Foundation, and the Illinois <br /> Attorney General Office through the Violent Crimes Victim Assistance Act <br /> (VCVA) <br /> - Additional funding is necessary to secure office space, pay for a phone and <br /> phone line designated for this program, and the printing of brochures and other <br /> program material. The bulk of the additional funding requested is to cover the <br /> cost of immediate needs for families such as shelter, food, and clothing as well as <br /> to pay for counseling services. <br />
