38. BILLING. Licensees hereunder shall bill their customers on a regular and <br /> consistent basis, but not more frequently than once every month. Licensees may use <br /> different methods for collection of payment of bills but, with the exception of credit card <br /> processing fees, licensee shall not charge different rates for in-person, by-mail, online or <br /> other payment methods. <br /> 39. RATE SCHEDULES. Monthly customer fees for garbage and recycling services <br /> shall only be those specifically set forth herein and shall include the cost of garbage, <br /> recycling, minimum landscape waste as described in this Chapter and its Rate Appendix, <br /> and for extra containers if so provided, and for other fees specifically included in the <br /> Promulgated Rules approved by the Office of the City Manager and posted to the City's <br /> official website. Licensed haulers are allowed to assess extra charges only for extra <br /> landscape waste removal services beyond the "Minimum Removal Residential Service" <br /> provided in this Chapter, and for large item waste removal services beyond those <br /> specifically stipulated in this Chapter. A schedule of those charges shall be filed with the <br /> Finance Department by each licensee, and shall be approved prior to their <br /> implementation by haulers by the City Manager as part of the Promulgated Rules. The <br /> process for any modification in the rates shall be contained in the Promulgated Rules. <br /> 40. UNIQUE SERVICES. Nothing in this Chapter shall prohibit a licensee from, by <br /> mutual agreement with a party served, establishing a price for unique or unusual, other <br /> than regular haulage of things other than refuse and recyclables including but not limited <br /> to back door service or for customized removal of refuse and recyclables in amounts <br /> greater than, or at frequencies more often than, that included in regular service for which <br /> rates are established hereunder. Back door service shall be provided to Hardship <br /> Customers at no charge. No unique services fees or charges will be assessed or collected <br /> for basic services as described in this Chapter. <br /> 41. ANNEXATION.... <br /> 42. REVOCATION OF LICENSE. The City Manager may suspend, revoke, fine, or <br /> deny renewal of a license to remove refuse and recyclables issued under the provisions <br /> hereof or of any other applicable code, ordinance, rule, regulation, or statute of the City <br /> of Decatur, County of Macon or State of Illinois, or seek alternative haulers, as provided <br /> in this Chapter. The fine shall not exceed Five Hundred Dollars ($500.00) for each <br /> offense, and each day of a continuing violation shall be deemed a separate offense. Such <br /> suspension, revocation, fine, or denial of renewal may take place only after the licensee <br /> has been given written notice warning of possible suspension, revocation, fine, or denial <br /> and a hearing, if requested by the licensee, before the City Manager. Such request for a <br /> hearing must be made within five (5)days of receipt of such written notice. If a license is <br /> revoked or suspended, a pattern of poor collection services exists in one or more <br /> neighborhoods of the City, or the licensee is otherwise unable to operate, after notice is <br /> provided to the licensee, the City Manager may assign the route(s) to another licensed <br /> hauler on a temporary basis. Alternatively, if a licensee is found to be unable to properly <br /> perform its duties, after notice is provided to the licensee, the City Manager may request <br />
