Exhibit A <br /> SCOPE OF PROFESSIONAL SERVICES TO PREPARE A <br /> STATE REVOLVING FUND (SRF) PROJECT PLAN FOR LOST BRIDGE <br /> NORTH (TA-3N) AND FLORIAN (TA_6) BASINS SEWER <br /> REHABILITATION <br /> SRF Loan Assistance Description of Work <br /> • Provide assistance and guidance to Decatur Public Works staff to submit a project plan report <br /> with the intent of obtaining an SRF Loan through the Infrastructure Financial Assistance <br /> Section(IFAS). The goal is for this sewer rehabilitation project(sewer mains, manholes and <br /> lower laterals)to be eligible for the Fiscal Year 2026 (starting July 1, 2025)Intended Use Plan <br /> (IUP)issued by IEPA—IFAS,typically in the month of June as a draft. <br /> • For the required project plan,revisit the Preliminary Opinion of Probable Project Costs <br /> (OPPC) from the 2017 SSAA and develop an updated OPPC. <br /> The Scope of Services for the Lost Bridge North and Florian Basins shall consist of the following tasks. <br /> 1. Funding Nomination Form (FNF). Prepare a project Funding Nomination Form meeting <br /> the requirements for obtaining an SRF Loan. Submit a Funding Nomination Form on or <br /> before March 31, 2025. Also included is preparing the Authorization of Representative to <br /> Sign Loan Documents form. <br /> 2. Project Plan Report.Prepare a project plan meeting the requirements for obtaining an SRF <br /> Loan. These requirements are found at 35 Ill. Adm. Code Section 662.320. The Project <br /> Planning Submittal Checklist shall be completed as part of Project Planning. Engineer will <br /> assist City with the completion of the forms required to be submitted in conjunction with the <br /> Project Plan,including the following. <br /> a. Project Planning Checklist <br /> b. Existing User Charge and O, M&R certification sheet <br /> C. Project Planning Verification of Cost and Effectiveness Analysis <br /> d. Preliminary Environmental Impacts Determination(PEID). <br /> e. Prepare updated opinions of probable project cost. <br /> 3. IEPA Loan Applicant Environmental Checklist. Obtain the required environmental <br /> signoffs (IDNR-SHPO, IDNR-Threatened & Endangered Species, US Army Corps of <br /> Engineers, IDNR-Office Water Resources, & IDOA) for the project and complete the IEPA <br /> Loan Applicant Environmental Checklist for City signature. Assist with preparing and <br /> obtaining the following: <br /> a. Project Environmental Impact Determination(PEID) Categorical Exclusion <br /> b. Advertisement of PEID for 10 day comment period <br /> Services to be performed by the City of Decatur <br /> 1. Public notifications. <br /> 2.
