Agreement No.25-0378-3192301 <br /> The Grantee shall design a sign which acknowledges the participating agencies and identifies Section 319 of the <br /> Clean Water Act as a funding source of the project. The Grantee shall complete and submit a sign design to the <br /> Grantor for review and approval by June 1, 2025. Upon Grantor's request, the sign design shall be re-submitted <br /> containing all required modifications by the Grantee to the Grantor for review and approval. The Grantee shall <br /> erect the sign prior to the installation of the BMP(s) and for a period thereafter as mutually agreed upon by the <br /> Grantee and the Grantor. <br /> The Grantee shall secure any necessary permits prior to the implementation of the Design developed under Item <br /> 2 of Exhibit D of this Agreement. In the event that the Grantee does not own the entire project site, the Grantee <br /> shall enter into legally binding agreements with participating landowners to ensure that the BMPs are maintained <br /> as designed and that the O&M Plans are implemented for no less than 10 years from the implementation of the <br /> BMPs. The draft landowner agreement shall be submitted by the Grantee to the Grantor for review by June 1, <br /> 2025. Upon Grantor's request, the Landowner Agreement shall be re-submitted containing all required <br /> modifications by the Grantee to the Grantor for review and approval. The Grantee shall submit a copy of the <br /> executed agreement(s) to the Grantor prior to the implementation of the BMP. <br /> Implementation of the BMP Designs developed in accordance with Item 2 of Exhibit D of this Agreement must <br /> begin by March 1, 2026. In the event that construction and implementation of the Designs developed in <br /> accordance with Item 2 of Exhibit D of this Agreement has not begun by March 1, 2026, the Grantee, or its <br /> Assigns, shall immediately discontinue all work on the Design implementation, unless an extension is requested <br /> by the Grantee and approved by the Grantor prior to January 15, 2026. In the event of such a discontinuation of <br /> work, limited costs incurred in association with the BMP Design will be eligible for reimbursement by the Grantor. <br /> The Grantee shall complete the implementation of the BMPs by June 1, 2026. Upon completion of the BMPs, <br /> the Grantee shall complete and submit Part II of the BMP Documentation Form with all supporting <br /> documentation to the Grantor by June 30, 2026. Support documentation includes invoice and photographic <br /> documentation. <br /> WATERSHED-BASED PLANNING STRATEGY AND SELF ASSESSMENT OF THE PLAN <br /> 3. The Grantee shall cause the development of a succinct Watershed-Based Planning Strategy (Strategy) to <br /> update the Big/Long Creek watershed plan as an Illinois EPA approvable Watershed-Based Plan (Plan) for the <br /> Big Creek-Long Creek watershed (HUC 071300060409) and Long Creek watershed (HUC 071300060406) of <br /> the Upper Sangamon River watershed. The Plan shall be consistent with US EPA watershed-based plan <br /> guidance found in Appendix C of the Nonpoint Source Program and Grants Guidelines for States and Territories <br /> dated April 12, 2013 (as revised) and Chicago Metropolitan Agency for Planning's "Guidance for Developing <br /> Watershed Action Plans in Illinois" dated June 2007. <br /> The Strategy shall identify any updates needed in the existing watershed plan. See Attachment 2 for required <br /> components of a Watershed Resource Inventory. The updated plan will be developed from the existing <br /> watershed plan with input from the local stakeholders and local experts. The Plan shall include a problem <br /> statement, goals, and objectives and shall identify and assess use impairments of water resources along with the <br /> causes and sources of such impairments. Potential management practices shall be identified for prevention, <br /> remediation, restoration, and maintenance to achieve water quality and natural resource objectives. The Plan <br /> shall identify the costs and funding sources associated with implementing recommendations of the Plan. The <br /> Plan shall include an implementation schedule; description of interim, measurable milestones; public information/ <br /> education component; and a monitoring component. The Plan shall contain tables identifying site-specific best <br /> management practices (BMP) recommendations along with the associated units (number, feet, acres) that <br /> should be implemented, cost of implementation, estimated pollutant load reduction, priority, and responsible <br /> State of Illinois <br /> GRANT AGREEMENT FISCAL YEAR 2025 <br /> Page 28 of 32 <br />
