Agreement No.25-0378-3192301 <br /> entity for each site-specific recommended BMP. <br /> The Grantee shall perform a separate self-assessment for the final Plan developed under this Agreement using <br /> Illinois EPA's Watershed-Based Plan or Total Maximum Daily Load Implementation Plan Joint Evaluation Form <br /> (Form) to verify that all elements of a watershed-based plan required in USEPA's April 12, 2013 guidance have <br /> been met. The Form shall be completed for the watershed-based plan and submitted by the Grantee to the <br /> Grantor by August 1, 2026 unless otherwise identified in the Strategy. Upon Grantor's request, the Form shall be <br /> re-submitted containing all required modifications by the Grantee to the Grantor for review and approval. <br /> The Strategy shall be submitted by the Grantee to the Grantor by January 15, 2025. Upon Grantor's request, the <br /> Strategy shall be re-submitted containing all required modifications by the Grantee to the Grantor for review and <br /> approval. Only minimal activities related to Watershed-Based Planning shall be started until the Strategy is <br /> approved by the Grantor. The Grantee shall complete the implementation of the Strategy by August 1, 2026. <br /> PROJECT REPORT <br /> 4. The Grantee shall evaluate and prepare a report on the success of the Lake Decatur Water Quality Initiative <br /> Phase 3 Project in terms of water quality and NPS pollution control. The report shall document the grant <br /> agreement number, the project period, project title, tasks, implementation schedule, budget, obstacles and <br /> successes encountered during implementation of the project. For all BMP implementation developed under Item <br /> 2 of this Agreement, the report shall include pre and post conditions, pollutant load reduction amounts for <br /> sediment, phosphorus, and nitrogen, the type and location of practices, plans and specifications, the O & M Plan, <br /> a description of installation and construction techniques, and materials used (including plant species). The <br /> Project Report shall be completed and submitted by the Grantee to the Grantor for review and approval by <br /> August 1, 2026. Upon Grantor's request, the Project Report shall be re-submitted containing all required <br /> modifications by the Grantee to the Grantor for review and approval. Upon approval, the Grantee shall submit <br /> two (2) paper copies and one (1) electronic copy of the Report to the Grantor. <br /> PERFORMANCE STANDARDS <br /> All products produced, and all work performed by the Grantee under this Agreement shall be subject to review <br /> and approval by the Grantor to determine eligibility and acceptability in meeting the terms and intent of this <br /> Agreement. <br /> The Grantee shall be responsible for the professional quality, technical accuracy, timely completion, and the <br /> coordination of all services furnished by the Grantee under this Agreement. The Grantee must, without additional <br /> financial assistance, correct or revise any errors or deficiencies in its services. <br /> The Grantee will perform such services as necessary to accomplish the objectives of this Agreement, in <br /> accordance with all the terms of this Agreement. <br /> State of Illinois <br /> GRANT AGREEMENT FISCAL YEAR 2025 <br /> Page 29 of 32 <br />
