APPENDIX A <br /> SUMMARY OF PRIOR COUNCIL ACTIONS <br /> • August 20, 2012—Council approved Resolution R2012-161 awarding the contract for the <br /> Franklin and Main Streets Reconstruction and Rehabilitation Project, City Project 2010-13, <br /> to Illinois Valley Paving, a Division of United Contractors Midwest, Inc. in the amount of <br /> $1,312,048.80. The resolution authorized an expenditure of up to $1,342,048.80 to allow <br /> $30,000 for project contingencies. <br /> • August 20, 2012—Council approved Resolution R2012-163 authorizing the purchase of <br /> various street light poles and luminaires from Sternberg Lighting to be installed as part of <br /> the Central Park Streetscape and Open Space Enhancement project. The Resolution <br /> authorized the expenditure of$174,934.30. <br /> • September 4, 2012—Council approved Resolution R2012-166 authorizing a contract in <br /> the amount of$1,735,022.08 for the Central Park Streetscape Enhancement Project, City <br /> Project 2012-14, to Otto Baum Company, Inc. as the lowest qualified bidder. <br /> • September 4, 2012—Council approved Resolution 82012-167 authorizing Change Order <br /> No. 1 to the Agreement with Phillip W. Cochran in the amount of$20,000 to provide <br /> construction engineering services for the Central Park Streetscape Enhancement Project. <br /> Change Order No. 1 was in addition to the previous Agreement amount of$82,411.57 for a <br /> total adjusted Agreement amount of$102,411.57. <br /> • March 17, 2014—Council approved Resolution R2014-17 appropriating $610,000 in <br /> Motor Fuel Tax Funds to pay the City's portions of construction costs and construction <br /> engineering costs for the Franklin Street Streetscape Project. <br /> • March 17, 2014—Council approved Resolution R2014-18 authorizing a Local Agency <br /> Agreement for Federal Participation with the State of Illinois for the Franklin Street <br /> Streetscape project to utilize $1,467,317 of Federal funds and $757,074 of City funds to be <br /> paid partially from MFT funds, the City's Bond fund and the City's General fund. <br /> • March 17, 2014—Council approved Resolution R2014-19 authorizing an agreement with <br /> Chastain& Associates, LLC for construction engineering services for the Franklin Street <br /> Streetscape project for a fee not to exceed $50,000. <br /> • May 19, 2014—Council approved Resolution R2014-46 accepting the bid and awarding <br /> the contract for the William Streetscape Enhancement project to Christy-Foltz, Inc. for <br /> their bid price in the amount of$121,853. <br /> • May 19, 2014—Council approved Resolution R2014-47 to appropriate an additional <br /> amount of$90,000 in Motor Fuel Tax funds to pay the City's portion of construction costs <br /> and construction engineering costs for the Franklin Street Streetscape Enhancement <br /> project. <br /> • May 19, 2014—Council approved Resolution R2014-48 authorizing an amendment to the <br /> Local Agency Agreement with the State of Illinois for the Franklin Street Streetscape <br /> Enhancement project for the utilization of Federal funds, City funds and Motor Fuel Tax <br /> funds to pay the City's portion of construction costs and construction engineering costs. <br /> Page 16 <br />
